1. Sponsor. Patient Now LLC, 6833 South Dayton Street #1016, Greenwood Village, CO 80112 ("Sponsor").
2. Eligibility. Open only to owners, operators and authorized decision-makers (for example, a medical director, practice manager or administrator) of a med spa, plastic surgery practice or wellness clinic operating in the United States (each, a "Practice") who are legal U.S. residents and have reached the age of majority in their state of residence (19 in Alabama and Nebraska; 21 in Mississippi). The Practice is the entrant and potential winner. By entering, you represent that you are authorized to enter on the Practice’s behalf. Current Sponsor customers are eligible. Not eligible: (a) employees, officers, directors, contractors, partners and resellers of Sponsor and their immediate family and household members; (b) employees or agents of competing vendors; (c) consultants or agents entering on behalf of a third party; (d) Practices that completed a Sponsor demo in the 90 days before October 19, 2026; and (e) any employee or official of, or facility owned or operated by, any federal, state or local government entity.
3. Promotion Period. Begins 12:00:00 a.m. ET on October 20, 2026 and ends 11:59:59 p.m. ET on November 6, 2026 (the "Promotion Period"). Sponsor’s clock is the official timekeeper.
4. How to Enter. There are two ways to enter. Each Practice is limited to one (1) entry in total, regardless of method, number of locations, attendees or products demoed. Multi-location groups under common ownership count as one Practice.
(a) Demo Method. Attend a live demo of any PatientNow product with a Sponsor representative that takes place during the Promotion Period. Scheduling a demo in the Promotion Period does not count if the demo takes place after November 6, 2026. Current customers must demo a product their Practice does not currently license.
(b) Free Online Entry Method. Visit [URL] and complete the entry form with your name, title, Practice name, Practice address, business email and business phone. No demo, meeting or purchase is required. Free entries must be received by the end of the Promotion Period.
Entries received by either method go into the same drawing and have an equal chance of winning. Incomplete, late, automated, duplicate or fraudulent entries are void. Sponsor is not responsible for lost, late, misdirected or incomplete entries or for technical failures.
5. Demo Disclosure. A demo is a sales presentation of PatientNow software (EMR, practice management, payments, marketing and AI tools for aesthetic and wellness practices) lasting approximately 30 minutes. Attending a demo creates no obligation to buy anything.
6. Drawing and Odds. On or about November 10, 2026, Sponsor will randomly select one (1) potential winner from all eligible entries received. Odds of winning depend on the number of eligible entries received. Based on an estimated 1,000 eligible entries, estimated odds are 1 in 1,000.
7. Notification and Verification. The potential winner will be notified by email and/or phone on or about November 2–6, 2026. The potential winner must respond and return a signed Declaration of Eligibility and Liability Release (and, where lawful, a Publicity Release) and a completed IRS Form W-9 within five (5) business days of notification. If the potential winner does not respond or comply, is found ineligible or the notification is undeliverable, the prize will be forfeited and an alternate winner may be drawn from the remaining eligible entries. Sponsor may verify eligibility, including practice type, business status and the entrant’s role.
8. Prize. One (1) prize of $5,000 USD, paid to the winning Practice by check or ACH within 30 days after winner verification, intended for use toward the Practice’s holiday party or staff celebration. Approximate retail value (ARV): $5,000. Total ARV of all prizes: $5,000. The prize is not transferable. No substitution or cash alternative is available except at Sponsor’s discretion with a prize of equal value. The winner is responsible for all applicable taxes. Sponsor will issue an IRS Form 1099 as required by law.
9. No Obligation; Entrant Compliance. Entering or winning does not obligate anyone to buy or use any PatientNow product. The prize is not conditioned on any current or future purchase, referral or business relationship. Entrants are responsible for making sure that accepting the prize complies with their organization’s policies and with any professional, ethical or regulatory obligations that apply to them.
10. Publicity. Except where prohibited by law, by accepting the prize the winner consents to Sponsor’s use of the winning Practice’s name, city and state in announcements of the promotion without further compensation.
11. Release and Limitation of Liability. By entering, entrants agree to release and hold harmless Sponsor, its parents, affiliates, subsidiaries and their respective officers, directors, employees and agents, as well as Meta Platforms, Inc. and X Corp., from any and all claims, losses or damages arising out of participation in the promotion or the acceptance or use of any prize.
12. Privacy. Information collected in connection with this promotion is subject to Sponsor’s Privacy Policy at https://www.patientnow.com/legal/privacy-policy. Entrants may receive marketing communications from Sponsor and may unsubscribe at any time.
13. Social Media. This promotion is in no way sponsored, endorsed, administered by or associated with Meta, Instagram, LinkedIn, X or any other platform.
14. General. Sponsor may disqualify any entrant who tampers with the entry process or acts fraudulently. If fraud, technical failure or any other cause beyond Sponsor’s control impairs the integrity of the promotion, Sponsor may modify, suspend or cancel the promotion and award the prize by random drawing from eligible entries received before the action. Sponsor’s decisions are final. Void where prohibited or restricted by law.
15. Disputes. These rules are governed by Colorado law. Any dispute must be resolved individually, without resort to any form of class action, exclusively in the state or federal courts located in Colorado.
16. Winner’s Name. To request the winner’s Practice name and state, email notice@patientnow.com or mail a request to Sponsor’s address above by December 31, 2026.
